CONFIRMATION OF REQUEST FOR LEAVE

This form should be completed by Parents or Legal Guardians Only.

Please note that this form does not guarantee permission for Leave, which remains in the hands of the Principal. It merely states that you are aware of the fact that your child is applying for Leave and that you approve of the destination which is indicated on the form. This Leave is for a child to go directly into the care of his host.

In requesting this leave, you acknowledge that you have been in contact with the nominated host and both parties are in agreement with the arrangements below.

Students are required to submit a written Application for Weekend leave by 6.00 pm Thursday evening with their parent’s signature on the correct leave form.
